Before starting the form, you may find it helpful to gather together full details of your child’s provision which could include names and levels of resources used, photos of their learning and any relevant professionals’ reports that have been written since you last updated us. If your child has an Education Health Care Plan (EHCP), you will need to refer to this as well.
