Development of Adult Social Care Gateway Services
Development of Adult Social Care Gateway Services – your views can help shape the future
Hertfordshire County Council is developing a model to support people’s access to Adult Social Care.
This gives us the opportunity to look at how individuals find information and advice about adult care and support services in Hertfordshire, and how they access Adult Care Services - which might include contacting us to arrange an assessment, to find support for carers or about care and support at home or in a care home.
We are gathering feedback from those who have been in contact or receive support from the Council, including people who have lived experience, carers and professionals, to help us improve the way we work with those making contact.
The feedback we get will be important as it will help shape our new ‘Gateway’ service and help improve the way people are able to get in touch with us.
To do this we are using a process called ‘Working Together for Change’. The approach uses information collected from local people to shine a light on what is working well, what is not working so well and what may need to change for the future, to ensure services are tailored to people’s needs and aspirations for the future.
A key part of ‘Working Together for Change’ is about involving people who receive services answering 3 questions, to tell us about the things that matter most to you in your life as a whole.
Your answers will help steer 2 workshop sessions, planned for 6th and 9th September this year, that will include people who receive Adult Care Services, voluntary organisations, and Council staff, to identify ways we can improve the local services.
Please indicate at the end of the questionnaire if you would like the chance to be involved in the workshop sessions.
If you would like more information about this questionnaire or would prefer to complete it by telephone or video call, request a print or braille version or in an accessible format please contact 01992 555354 or CLGP@hertfordshire.gov.uk.
If you are Deaf BSL User, you can use our SignVideo facility by going in SignVideo app, click on free directory and select Hertfordshire County Council. Please ask for Nadine at the Information and Support Gateway Programme.
The closing date for the questionnaire is 5 August 2022.
