We are sorry to hear you wish to leave Hertfordshire Music Service; if there is anything you would like to discuss with us before you complete the request form please contact us on 01438 844851 or your music centre directly.

Please use this form to withdraw from your music lessons or ensemble membership. Written notice for any withdrawals or changes to HMS tuition must be received by the beginning of the upcoming half term break (i.e. if you wish to withdraw and or make changes for the start of the spring term notice must be received by the autumn half term break) – Hertfordshire term dates.
