SEND IASS parent survey

If you have accessed the SEND IASS service in the past 12 months, then we kindly ask if you can spare a few minutes to complete this survey.

The data on this form is being gathered for the purpose of obtaining feedback on the Special Educational Needs and Disability Information Advice and Support Service (SENDIASS). SENDIASS are a statutory service commissioned by Hertfordshire County Council. They work at arm’s length from the Local Authority providing impartial and confidential information, advice and support in relation to children and young people with SEND aged 0-25.

Hertfordshire County Council will use this information to review the SENDIASS service. This information is being gathered and will be processed on the basis that you are giving consent for us to do so. If you wish to withdraw consent at any time you can do so by emailing SENDtransformation@hertfordshire.gov.uk.

If applicable – the information you have provided to us will be used alongside additional information obtained from other SENDIASS stakeholders.

The information you give us will be held by the SENDIASS team and the department  of Children’s Services in Hertfordshire County Council. It will only be used to contact you if you have any complaints we can improve on. 

We will not share this information with a third party or use it for any other purpose than the review of the SEND IASS survey.
