Great, you're quitting! To get signed up, please have the following information ready:

• Your name and contact details
• Your baby's estimated due date (EDD)

And if someone is joining you for the 'Family Quit' programme:

• Your support person’s name and contact number (if they're ok with you sharing this).

When you've submitted your form, someone from your maternity unit or our health service will contact you. They'll guide you through the next steps and answer any questions you have.
