SEND stands for Special Educational Needs and Disabilities. Children and young people with SEND need some extra support to learn and grow. This survey will help plan services and support for children and young people with SEND aged 11-25 in your local area and will help your teachers plan work in schools/colleges.
Your parents, carers, youth worker/advisor, school or college can help you answer the questions.
If you are uncomfortable with any question, you can choose not to answer it.
